Leadership

Fractional CMO leadership tailored to your specific needs

A practical way to define the mandate of a part-time chief medical officer before the engagement starts.

Part-time clinical leadership works when the mandate is clear and fails when it is vague. The following distinctions help define the arrangement:

Decisions to own, to advise on, and to leave alone

List the clinical decisions that will arise in the next year: indication choice, protocol approval, dose selection, go/no-go at a readout, amendments, vendor changes and agency strategy. For each, agree whether the CMO decides, recommends or is consulted. Commercial, financing and corporate decisions generally stay with the CEO and board; the CMO’s job is to make the clinical input to them reliable.

Governance that survives part-time attention

Set up decision records, a regular clinical review meeting and clear escalation routes, so that issues do not wait for the CMO’s next available day. A part-time leader is most effective when the structure around them keeps working in their absence.

Working with the CRO and other providers

Make clear who gives direction to the CRO, the medical monitor and other providers, and who resolves disagreements among them. Confusion about authority is a frequent source of delay in outsourced programs, and it is cheaper to settle at the start.

Board and investor contribution

Agree what the CMO will present, how often and in what form. A short, honest clinical update that names uncertainty is worth more to a board than a polished one that does not.

Time, availability and limits

Be realistic about what a given time commitment buys. A fixed number of days a month can cover governance, review and a few key decisions. It does not cover round-the-clock safety coverage. If continuous medical monitoring or a formal officer title is needed, treat it as a separate question with its own review.
